I just inherited a site with the Ultimate Locator installed. My coworkers have pointed out to me that it is currently "broken" and needs to be fixed ASAP!

Unfortunately, I've never used UL before, and while I'm sure I'll get the hang of it, I currently need for it to be fixed ASAP - and I can't even tell what's wrong!

Basically all I'm getting is "Undefined Variable" errors, followed by what I guess is the proper output. Please check it out at http://www.eyelinegolf.com/dealer_locator.php and let me know if it's something you think I can fix!
